The Distinguished Hospice Interdisciplinary Team Member Award recognizes individuals who exemplify excellence in hospice and palliative care through high-quality service, innovative programming, and an unwavering commitment to the practice of palliative care in a hospice setting.

Who Is Eligible?
All members of the hospice interdisciplinary team are eligible to be nominated. While AAHPM membership is not required for nominees, the nominator must be an AAHPM member.
How to Apply
Submit your nomination before Thursday, August 13, 2026, at 11:59 p.m. PT. All eligible nominations will be reviewed by the AAHPM Awards Committee for consideration and awards will be announced in early November 2026.
