Become Certified in 2025

Attention hospice physicians! The initial application is now open through July 31, 2025. Highlight your knowledge as a hospice physician and join the HMDCB community by earning your HMDC credential this year. Make sure to apply early to SAVE in late fees!

Early bird application window: January 27–April 30, 2025 | $1,250

Regular application window: May 1–July 1, 2025 | $1,350

Final application window: July 2–July 31, 2025 | $1,450

Exam Window: April 7–September 25, 2025

Employer Discount Program

Discounts are available for organizations that want to certify multiple physicians during the 2025 initial cycle. Discounts are available for the application and advertisement/sponsorship opportunities. Renew Your HMDC Credential

If your credential expires this year, make sure to complete the Continuing Certification Program (CCP) by October 31, 2025. The CCP includes an annual longitudinal assessment that is open book, is untimed, and can be retaken as needed to ensure you understand why the right answers are right. HMDCB is committed to your ongoing learning and success, and that’s why the CCP provides opportunity for continued learning while also assessing the competency, skill, and judgement of certified physicians. HMDCB Community Events

All certificants are invited to attend the 2025 HMDCB community events. These events are a great place to connect with your hospice peers and expand your hospice knowledge.
